Patron Registration Policy
 

Patrons must fill out a registration form to receive a library card.  Children who are five years old or are able to write their own name may get a library card.  A parent or guardian must fill out the registration form (blue) and sign it to indicate that he/she will be responsible for all materials checked out on the child's card.  Patrons sixteen or over must fill out the registration form for an adult card (white.) 

There is a $2.00 replacement fee for lost or damaged card.
